GOD BE WITH THE DAYS WHEN MEDICINE WAS A VOCATION!

I have spent 3 days trying to see my GP. I was told I had to telephone at 8.30 am yesterday morning. I rang 45 times between 8.30 am and 9.20 am.

When I finally got an answer I was told I had missed all the doctors “call you back” spots for the day.

I insisted to the receptionist that I needed to see a doctor.

In the afternoon I got the flowing message from a number I could not reply to!

Dear Mr Buckley,

Following your medication request today the GP feels this would be best being discussed with our Nurse Mandy who will call you tomorrow to discuss your symptoms, please expect the call.

Dear Dr Hopkins,

I have been a patient of this practice for 38 years. I have never done anything but respect the practice. I have never asked too much or bothered the doctors unnecessarily.

I am now a 70 year old man with various chronic medical conditions.

I have been telephoning the surgery for three days. This morning I called 45 times from 8.30 am to 9.20 am. When answered I was told I had missed the list for todayā€™s calls from a doctor.

I am absolutely disgusted by my treatment today.

But that will not bother you, I imagine?

When I first came to Larne in 1984 we had 14 GPs in the town.

Not only did they do five 8 to 6 days – they also covered their own overnight cover.

On top of that they manned the 24 / 7 / 365 A&E in the town.

Today the GPs do a four and a half day week, have all other cover performed by an out of hour doc service and have no A&E work to perform.

And, unlike in the old days, they have practice managers and a whole plethora of office staff – and earn Ā£ 130,000 plus per annum – and Covid and other bonuses.

Professor John Hermon Taylor Dean of Surgery St George’s Hospital London. Renowned surgeon and Crohns Disease researcher. Still working in his mid 80s.

Dr Tom Trouton cardiologist N. Ireland. Always gave and still gives 1000%

Pat. I donā€™t believe you should have a right to medical care. If you want to see a GP you should pay to go private or stay quiet and wait for the NHS to get to you it itā€™s own time. It is free after all!

With any luck, the NHS will be privatised. I love the idea of it being taken over by Russian owned Gazprom: ā€œyou want sex change operation? ā‚¬4000. With anaesthetic ā‚¬6000!ā€

Seen as you asked….I think you need to find a new Doctor. I’m little over 38 but my family have had the same and his father and soon to be a son in line. I have my GP’s mobile. I used it once or twice. And his practice have so many patients I would not like to mention the figure. And most are lifelong patients and families and probably most on HSE (ROI). I know this is not the case with most people, yet most do take up all of doctors time. I understand that you do not need servicing frequently, but maybe they simply do not like you after all these years, maybe political or religious or other reasons? Just a thought but find a new GP. And not necessarily an Irish one as there are many around seeking patients. ?

The most famous was the echo;; of Salerno in Southern Italy, reputedly founded by a Christian, an Arab, and a Jew. A health spa as early as the second century, Salerno was surprisingly free of clerical control even though it was very close to the famous and very powerful monastery of Monte Cassino. The medical faculty at Salerno allowed women to study there.
Bishop have the RCC got to your doctor ??? I ask the question with humour, you speak of medicine as a vocation. Vocation is a clerical idea, there is in fact no such thing. It was conceived to fortify the caste of Cleric, Medic and Legal; to keep all “not called” out. I assure you your doctor will see you if you offer 100 pounds cash.

Absolute nonsense! It is far from free ,in the UK each working person pays a percentage of their earnings to National Insurance (N.I) deducted from their salary.Medical treatment is “free” at the point of source, in other words residents in the UK do not pay when in hospital or, after medical treatment as is being deducted via N.I. Dental treatment is only free in very few circumstances, dental treatment can be access at a little lower cost if a dentist is a NHS one, dentures provided by this dentist are of very poor quality and, break easily.
People requiring medication in England have to pay for their prescription unless they are exempt due to the age, income or medical condition. You can get a free prescription if you are: Aged 60 or with some medical conditions. Each individual item costs Ā£9.35p , so, e.f five prescribed items cost Ā£45-75. If there is a chronic problem an annual cost can be used which greatly reduces expenditure, it costs around Ā£110-0p, some chronic problems such as a diabetic get there medicine at no cost from the pharmacy.
It is a very good system but, has it’s flaws, as Pat has pointed out many young doctors don’t waste to be in G.P practice., or only work part time.

Itā€™s the same in all public services. Donā€™t expect the Land Registry, HMRC, The Probate Registry or The Office of The Public Guardian to answer the phone in under 40 minutes. Read the Diocesan privacy policy. For example one way a Diocese gathers
information on a person is through the parish or other third party! You might want to know what information is on your files held by your doctor, dentist, solicitor and neurologist and Diocese particularly if you had to take a legal case- for example if medical malpractice occurred. What if a priest abused you or a son or daughterā€¦? You are either very naive, very trusting of all professional, or very stupid. You have a right to know what information is held about you on file.

I do not think you need to worry. Bishop Pat contacted an Irish Bishop on behalf of a vulnerable family member in a property matter and the only people that would have any right to know about that is the Bishop of that Diocese, Bishop Pat and the legal Guardians and representatives of that vulnerable person. It would have no legal or legitimate information gathering function in the running of the Diocese unless there is any conflict of interest we have not been made aware of by that Diocese. Pat, your experience is replicated around the province. An article in July in the Telegraph indicated that only 25% of gps work full time in the UK. This is having a severe impact on patients and putting an added and ongoing stress on A&E facilities everywhere.
The laugh is that money appears to be part of the reason for this due to pension regulations, but with gps earning Ā£120k pa, this gives the impression theyā€™re in it for the money not to provide adequate patient care.
Our local practice in Co. Derry is opened four and a half days a week and when you ring and eventually get through, you are faced with office staff who triage your call. This is a total cop out by the gps.
My wife had a rare stroke over 2 years ago and when my daughter rang to speak to a gp, the receptionist refused to allow her to speak directly to a gp. She was a medical student at the time and now a trained doctor. This condition is time critical and because it presented as atypical, we wanted to have a quick discussion just to confirm the diagnosis. The doctor rang back 45 mins later and gave off to my daughter about her ā€œdirectā€ approach to their triage expert.
Long story short, my wife went to A&E, was misdiagnosed with migrainous vertigo, sent home the same day despite worrying symptoms. She only got a proper diagnosis 3 months later following a MRI, simply because we pushed the issue.
Thankfully, she has recovered but our experience particularly with frontline gp service and A&E was exceptionally poor.

The arrogance of your reply is probably from a typical entitled smart arse. Most people cannot afford to take out private medical care. Each of us pay into the system with national insurance contributions. The least you expect is a basic provision of care which in increasing circumstances doesnā€™t happen.

Your experience + Pat chimes exactly with mine. You phone for an appointment, wait 30 mins / 45 mins in the queue to be told that all appointments have gone, and that you will have to phone back this afternoon, or tomorrow. So, you do, only to be told that all the appointments have gone, please phone back tomorrow morning…..and so on.
If you turn up in person at the surgery to ask about an appointment, the overwhelming sense from the receptionists is that you have an impertinence to be there and to expect anything.
When I made a bit of fuss, I was informed that this was not private health care and I could not expect the kind of treatment that I wanted. To which my response was, that this is my private health care plan because I have paid in to it for the last 55 years with my taxes and NI contributions ! Bloody cheek of them !
Remember, in the UK GP practices are a business owned by the GP partners, and they have a vested interest in making as much money as they can. So, they trim accordingly. To hell with the convenience of their patients as long as the GPs are making money ! I have noted that occasionally the GPs are contacting me to make an appointment for a blood test, cholesterol testing, blood pressure testing etc. I’ve worked out that the reason they are contacting me is that there is a fee that they receive when they hit targets so are keen to get me ticked off. But, if I decided I want to see the GP myself for my own reasons, sod that ! I’m costing them money so they are not bothered.
I’ve noticed also that the majority of GPs in my practice only work part time. It’s clear that what they are earning by working only part time is still more than enough to live in the nice houses in the nice neighbourhoods, still enables them to drive their Volvo cars, to send their kids to the nice fee paying schools, and still to go on their skiing holidays and summer holidays. Oh, and don’t forget the ‘conferences’ they are able to go on paid for by drug companies whose products they prescribe to us. When we eventually can see them !

I concur with everything you have said. You have to ring up in the morning between 8.30 am – 9 am, wait ages to then be told ā€œIā€™m sorry my love all the telephone consultations have gone for today, ring back tomorrowā€. So you ring back the following day and the same thing happens etc etc. No longer able to book an appointment to see a GP (itā€™s been like this since Covid). When you ask if you can see a GP in person the reply is ā€œSorry my love itā€™s emergencies onlyā€.
When I was a child in the 1960ā€™s, the GPā€™s surgery hours were printed outside the surgery and you just turned up and waited in the waiting room, morning surgery and afternoon surgery. Only one doctor who did all his own visiting and night time visits.
Itā€™s the same trying to get an appointment with a dentist – unless you are a private patient.

Nearby me there have sprung up two private GP practices. It’s nice bit of town, so I guess they are cashing in on the population here deciding to bypass their NHS GP and go private. At least they will get seen for a Ā£150 fee. However, not everybody can do that. So, we are definitely heading more in to a two tier system, which will become like dentistry where you get seen if you are private, if you are NHS you wait, and wait, and wait……

I agree there are many Harridans manning the G.P. surgery receptions.
As you can see from the figures above there are more female G.P.’S than men, the many of the women work part time, which is their RIGHT to choose. There is only so much that can be done in there paid time, I am sure many work far more hours than they are paid for. My surgery has improved by allocation of some of the less pressing appointments to the highly educated and skilled nurses, which relieves pressure on the doctor, also a very skilled phlebotomist. Nurses carry out cervical smear tests, etc.
Yes, they are well compensated for their work, but, for their responsibility, no money compensates, one slip-up and, their out, and their name is mud. Yes as in other professions they attend conferences, and rightly so, keeping up to date is of vital importance.

First of all, nurses are not well compensated….second of all, nurses are not well compensated…..and yada yada………………nursing is a true vocation and I commend every single one of them, overworked, not paid half their worth, and have the respect of very few governments.

Pat, you must be well aware of the pressures that the NHS is under. You must also be aware that GPā€s the country over are overwhelmed by the queues at their surgeries. Covid has slowed down the doctor/patient interaction and now we can only see our GP by appointment. Itā€™s inconvenient and very annoying but Iā€™d always give preference g
to someone definitely worse than me. When we feel vulnerable because of our sickness, condition or relentless pain, itā€™s very frustrating not to get to your GP or to have to wait in waiting room. My GP is and always has been superb to meā€¦However I would never feel so ā€œentitledā€ as to expect preferential treatment. If a priest had written this kind of letter of complaint, Pat would crucify him with a merciless onslaught. There is a sense of entitlement which Pat has appropriated to himself and it smacks of rank hypocrisy. None of us want a to feel we are just a ā€œnumberā€ to any doctor. I wish Pat and all who are unwell a good recovery..

Pat you may get an email from the Practice Manager saying Trust and Confidence in their Practice with you has broken down due to your letter.
The Best interests of both of you will be for you to seek another Practice.
People in the UK have been getting letters like that and when they seek another Practice their is none so NHS board appoints you one and they get more money.
You should not have written but gone to A&E or call 111 it is 24/7

Agree 100% Pat . Same story down south. The difference is in years gone by GPā€™S Pharmacists , Nurses , nuns etc went beyond the call of duty to look after people. These days , they are not prepared to do that. They are way more concerned about looking after themselves and their families. Maybe they are right. Itā€™s a me me world.

Maybe itā€™s best if you ring 111. That is what I have decided to do in future if I canā€™t get at least a telephone consultation after three attempts. At least that way you can tell 111 operator that you have tried three times to see a GP at your own practice but to no avail.

Having worked in specialist mental health services I can confirm that needs get moved around the system rather than being at the right level of care.
This is because health professionals try to take the stress off their own overworked area.
The ONLY solution to the NHS is large investment of money, people and everything. In reality the cuts have given us much less of a service than we used to have.
The other reason it needs a large investment is that it was never properly financially planned in the first place. They thought it would need one large investment and then everyone would be healthy and it would cost less to run as it went on. In reality it costs more because increasingly life expectancy increases the diseases of old age. Also when people were very pleased at their ā€˜freeā€™ dentures in the forties nobody could foresee the expensive procedures the NHS would now be doing.

The solution is very simple: tax billionaires hugely.

It is just the same in England and Wales. There should be weekend surgeries; nurses doing much more work and prescribing; triage for all patients arriving at a surgery; longer time limits for consultations with patients who need them; and we need many more doctors in training. BTW Nicky Hudson is yet to be announced as Bishop for East Anglia. He will love being near and may move to Cambridge. Auxillaries are yet to be appointed in Southwark and Wesminster. Then there is Plymouth.

Iā€™d venture a guess that it has to do with the fact that you were able to publicise it on this blog, and somebody has informed them. They donā€™t like negative publicity. Maybe they, and the bodies that supervise them, will take a look at all the other comments, which give a picture that is not flattering, and take note. People are not happy with GPs as currently run, by and large.

I have a new gp with a couple of months she’s lovely but can’t seem to get through to them they never answer the phone and they are so busy I’m expected to wait a couple of days for an appointment.
Your alot nicer than me pat, I tore him my old gp a new asshole over email, he hates confrontation, so I confronted him with plenty. he was useless.
I couldn’t get a new gp in town ,I comtwcted loads, none would take me on all doctors were full to capacity , I had to write to the hse they assigned one to me.
Doctors don’t care anymore, all lawsuits and over whelmed, it’s a bloody joke for those of us that have health problems

GPs dont have to give a reason for delisting a patient; they just tell him to find another doctor. Thatā€™s it. They can actually delist a patient just because they donā€™t like him.

Patients actually have very few rights. Power is in the hands of GPs to act almost at will. And some of them do. Itā€™s a form of abuse, bullying, and it needs to end.

I once made a complaint about a former GP over seriously incorrect information in my medical notes; the matter ended up in court with funding from the Equality Commission. The entry had been made by an idiot locum, and my GP told me she was powerless to erase the the errors.

After the court hearings, my GP wrote to me and advised that it might be in my best interest to find another doctor. My best interest? My freakinā€™ eye! šŸ˜• It was her interest she cared about, not mine; this had been the case all along. She just wanted rid of me, but couldnā€™t force me out of the practice, because GPs cannot delist a patient for making a complaint. I ignored the letter and refused to go. It must have annoyed the hell out of her, Iā€™m delighted to say.šŸ˜… She is now retired, and Iā€™m still with the practice.

I was in Galway University hospital the other day it is much similar to up north. There seemed to be more doctors and nurses having breaks and walking around with very few patients. When one enters the doors one is met with a huge amount of LGBT bunting and flags. One priest told me he feels intimidated when he goes to visit a patient with the flags in his face. I know it is not catholic and is state owned but why do we need this in our face. The parking charges are extraordinary. I can go on but another day..

It is a bit of post code lottery.
In London, on the phone you are no. 48 in the queue at 8.00am, hang on for an hour (costing a fortune) to find all appointments are booked.
Even the hospital consultants (some of them) are not much better. Telephone consultation on the phone with someone with ear problems. I ask you?
Scotland, GP surgery, on the phone, no. 3 in the Queue and sorted in 15 minutes. Appointment with GP in a week and at hospital a week later and prescription issued.
You are correct, it is no longer a vocation. It is a career and a lazy one at that.
Too much change and upheaval over many years in the NHS. Too much empire building and not enough patient care. Too much change is as bad as no change.
Please get well. Disagreements can be healthy.
